Heart Disease: Stents, Bypass Surgery Provide No Benefits To Stable Patients In Large Funded Trial

Stent in Coronary Artery“For patients with severe but stable heart disease who don’t want to undergo these invasive procedures, these results are very reassuring,” said David Maron, MD, clinical professor of medicine and director of preventive cardiology at the Stanford School of Medicine, and co-chair of the trial, called ISCHEMIA, for International Study of Comparative Health Effectiveness with Medical and Invasive Approaches. 

“The results don’t suggest they should undergo procedures in order to prevent cardiac events,” added Maron, who is also chief of the Stanford Prevention Research Center.

Patients with severe but stable heart disease who are treated with medications and lifestyle advice alone are no more at risk of a heart attack or death than those who undergo invasive surgical procedures, according to a large, federally-funded clinical trial led by researchers at the Stanford School of Medicine and New York University’s medical school.

The trial did show, however, that among patients with coronary artery disease who also had symptoms of angina — chest pain caused by restricted blood flow to the heart — treatment with invasive procedures, such as stents or bypass surgery, was more effective at relieving symptoms and improving quality of life.

Health Reports: CDC Finds That 35,000 Americans Die Of Antibiotic-Resistant Infections Each Year

CDC Antibiotic Resistance Threats in the United States 2019More than 2.8 million antibiotic-resistant infections occur in the United States each year, and more than 35,000 people die as a result. In addition, nearly 223,900 people in the United States required hospital care for C. difficile and at least 12,800 people died in 2017.

Germs continue to spread and develop new types of resistance, and progress may be undermined by some community-associated infections that are on the rise. More action is needed to address antibiotic resistance. While the development of new treatments is one of these key actions, such investments must be coupled with dedicated efforts toward preventing infections in the first place, slowing the development of resistance through better antibiotic use, and stopping the spread of resistance when it does develop to protect American lives now and in the future.

CDC’s Antibiotic Resistance Threats in the United States, 2019 (2019 AR Threats Report) includes updated national death and infection estimates that underscore the continued threat of antibiotic resistance in the United States. New CDC data show that while the burden of antibiotic-resistance threats in the United States was greater than initially understood, deaths are decreasing since the 2013 report. This suggests that U.S. efforts—preventing infections, stopping spread of bacteria and fungi, and improving use of antibiotics in humans, animals, and the environment—are working, especially in hospitals. Vaccination, where possible, has also shown to be an effective tool of preventing infections, including those that can be resistant, in the community.

Exercise Studies: Older Adults’ “Fragmented Physical Exercise” Linked To Higher Death Rates

Fragmented physical activity, but not total daily activity, was significantly associated with death, the researchers found. For each 10% higher degree of activity fragmentation, mortality risk was 49% greater. The duration of physical activity also mattered: Participants who frequently engaged in short bouts of activity of less than five minutes were more likely to die than those whose activity bouts lasted five minutes or longer. These associations held after adjusting for age, sex, body mass index and other factors.

Daily physical activity, which benefits health and quality of life, typically decreases in older adults. A new study shows that higher fragmentation of that activity — more transitions from activity to sedentariness — may be a better indicator of risk of death than a person’s overall level of physical activity.

Researchers from the Johns Hopkins Bloomberg School of Public Health, Baltimore, analyzed data from the NIA-supported Baltimore Longitudinal Study of Aging (BLSA), looking at a week’s worth of minute-by-minute physical activity for 548 healthy participants age 65 and older (average age, 76).

Cancer Studies: Vitamin D Limits Melanoma Growth By Boosting Effects Of Immunotherapy

To answer this question, researchers at the University of Leeds scrutinized the interaction between vitamin D and the vitamin D receptor (VDR) on melanoma cells. The researchers, fully aware that vitamin D on its own won’t treat cancer, hoped to identify cell signaling pathways that could lead to new therapeutic strategies.

Health Studies: Chronic Inflammation Reduced By Exercise, Lowering Risk Of Cardiovascular Disease

Massachusetts General Hospital“This study identifies a new molecular connection between exercise and inflammation that takes place in the bone marrow and highlights a previously unappreciated role of leptin in exercise-mediated cardiovascular protection,” said Michelle Olive, program officer at the National Heart, Lung, and Blood Institute Division of Cardiovascular Sciences. “This work adds a new piece to the puzzle of how sedentary lifestyles affect cardiovascular health and underscores the importance of following physical-activity guidelines.”

Scientists at Harvard-affiliated Massachusetts General Hospital (MGH) have identified a previously unknown biological pathway that promotes chronic inflammation and may help explain why sedentary people have an increased risk for heart disease and strokes.

In a study to be published in the November issue of Nature Medicine, MGH scientists and colleagues at several other institutions found that regular exercise blocks this pathway. This discovery could aid the development of new therapies to prevent cardiovascular disease.

Health Studies: People With Depression (Genetic) Reduce Depressive Episodes With Exercise

However, more physically active patients at baseline were less likely to depression, even after they accounted for genetic risks.

Increasing physical activity could pay dividends for people with a high risk of developing depression.

A team from Massachusetts General Hospital (MGH) recently discovered that several hours of weekly exercise result in a decreased chance to be diagnosed with a new episode of depression, even in patients with a higher genetic risk of developing Major Depressive Disorder (MDD).

The team examined the genomic and electronic health record (EHR) data of approximately 8000 patients in the Partners Healthcare Biobank, which represents the first study to show how physical activity influences depression despite genetic risk.

Sleep Studies: Increase In Preventable Diseases Tied To Disrupted “Circadian Rhythms” (Harvard)

This can lead to nighttime eating, which can influence the way the body processes sugar and could lead to a higher risk in diabetes. “What happens when food is eaten when you normally should be fasting?” Scheer asked the audience. “What happens is that your glucose tolerance goes out the window….So your glucose levels after a meal are much higher.” This can increase people’s risk for diabetes.

Frank Scheer, PhD, professor of medicine at Harvard Medical School and director of the medical chronobiology program at Brigham and Women’s Hospital in Boston, says disruption of the body’s circadian rhythms may be one major reason why more Americans are living with preventable diseases. During his keynote talk at the 2019 AAST annual meeting in St. Louis, he outlined how recent research supports the hypothesis that higher rates of shiftwork and other forms of nighttime disruption could be contributing to increased rates of obesity, diabetes, and other common ailments.
